This recent progress in basic, translational, and clinical research not only motivated the initiation of numerous new research initiatives but encouraged researchers to investigate entirely new field in order to gain a more systemic, if not holistic view on neurovascular and neurodegenerative diseases. Finally, the breakthrough that mechanical recanalization brought for acute stroke treatment impressively illustrated that major success in augmenting our arsenal to fight devastating neurovascular and neurodegenerative diseases are definitely possible. However, the last two decades were characterized by a tremendous progress in understanding the neurobiological and systemic pathomechanisms behind those diseases, and important reasons for previous failures in translating treatment concepts into the clinics have been unrevealed in parallel. All types of stroke, cerebral small vessel disease, and Alzheimer’s disease account for a significant proportion of serious physical and mental impairment cause a significant loss of quality of life and result in enormous socio-economic burden.
